Group Health presently has two questionnaires which are being used.
The Group Health Behavioral Health Care Management Outcome Questionnaire is to be used for patients in the Group Health Behavioral Health Care Management Program. This form has two parts. The first part assesses patient self-report of mental health symptoms, functioning, and substance use status and is administered at the beginning of the session. The second part assesses patient self-report of therapeutic alliance and is administered at the end of the session.
The Group Health Behavioral Health Adult Outcome Questionnaire is to be used for adults and adolescents over the age of 16 who are involved in outpatient behavioral health treatment. The questionnaire has two parts. The first part (outcome tool) is the PHQ-9 questionnaire with the addition of one question regarding substance use. This part of the tool is administered at the beginning of the session. The second part of the tool assesses patient self-report of therapeutic alliance and is administered at the end of the session.
